"I Had Nose Surgery, but My Nasal Tip Is Drooping Again"… The Cause Was the 'Muscles'
Kowon Plastic Surgery's Dr. Kim Hyung-taek says a nasal tip that droops again after rhinoplasty can be caused by nasal tip muscles, and that correcting them along with the cartilage helps.

Many people find that, as time passes after rhinoplasty, their nasal tip sinks back down. Dr. Kim Hyung-taek of Kowon Plastic Surgery points to the "nasal tip muscles" as the cause. Dr. Kim explained, "Raising the nasal tip alone is not enough. If the underlying cause of the drooping remains, the nasal tip is pulled downward again every time you smile, and over time a pollybeak ('arrow nose') or long-nose look can return." The key is the depressor septi nasi muscle. This muscle pulls the nose downward, making it look long and droopy and giving it an older-looking appearance.
The levator labii superioris alaeque nasi muscle, meanwhile, spreads the nostril wings sideways when you smile, making the nose look larger. Along with correcting the nasal tip cartilage, Kowon Plastic Surgery focuses on correcting these muscles as well, so that the nasal tip stays natural-looking even when you smile. However, results may vary depending on each person's nasal structure, so a thorough diagnosis is recommended.
